Healthcare Transformation Consultant

Position Description: Consultants are highly visible, hands-on personnel responsible for assisting physician offices with achieving Patient Centered Medical Home model recognition, managing provider metrics, workflow optimization and much more. These individuals are responsible for managing all aspects of client engagements to ensure project timelines are met and the final deliverable/product meets quality (and time frame) expectations. In addition, Consultants are responsible for business development activities including client retention, consultative selling to increase active client base and support the organizations Fee for Services (FFS), and other revenue generation activities.

Job Functions and Duties: Manages client engagement cycle from sales/project onset to successful completion through a well-defined work plan and goals (for each client) and with clearly defined expectations for meeting project targets and completion dates.

· Develops work plans and project administration tasks

· Prepares and presents project results and educates providers/practices appropriately

· Reviews documentation and helps practices prepare for program audits

· Supports client interfacing with public health agencies

· Stays current with regulatory changes and healthcare trends and educates clients on these changes in order to maintain his/her trusted advisor role

· Meets with assigned supervisor at least monthly to assess goals and barriers for projects and to gain feedback and recommendations

· Coordinates and manages project activity and resources for assigned providers/practices

· Prepares and updates weekly report linked to performance dashboard for supervisor review

Knowledge, Skills and Abilities:

· MUST HAVE experience working in or with a Patient Centered Medical Home and experience with practice transformation or process improvement

· Knowledge of the Patient Centered Medical Home model, healthcare revenue cycle and Alternative Payment Models is preferred

· Knowledge of quality improvement methodologies

· Knowledge of EHR adoption and implementation, as well as MIPS/Meaningful Use, is required

· Knowledge of clinical operations is preferred

· HEDIS knowledge required with Medicare Advantage experience desired

· Relevant healthcare industry, clinical, sales, or consulting experience is desired

· Possesses strong project management skills such as organization, planning, prioritization, and time management

· Can function effectively in a high intensity, constantly changing environment

· Works well under pressure and can meet deadlines

· Is self-motivated, proactive, and able to handle the autonomy/accountability associated with working remotely as well as independently

· Possesses exemplary interpersonal and communication skills, including public speaking

· Can consistently demonstrate well-developed written and verbal proficiencies and professional responses

· Possesses knowledge of Salesforce’s CRM application, or similar project/customer relationship management tools, and Microsoft Outlook, Excel, PowerPoint, Project, Visio, and Word

· A thorough knowledge of medical terminology and the ability to use it appropriately is desired

· A Bachelor’s Degree in Health Informatics and Information Management, Health Services Administration, Health Sciences or a related field of study is preferred; or multiple years of experience in the healthcare field.
